Casualty
Casualty is a weekly medical drama following the personal and professional lives of emergency room staff at a busy UK hospital. With nearly four decades on air, it balances urgent cases with deeper character arcs, offering authentic hospital realism mixed with genuine human drama. It's for anyone who enjoys ensemble casts, high stakes, and the messy lives behind the scrubs.
Should you watch it?
Watch it if you like a long-running hospital soap where the medical cases and the staff's private lives tangle together week after week, and you enjoy watching a familiar cast grow older on screen.
Skip it if you want a self-contained story with a clear arc, since this is a sprawling serial with nearly fifteen hundred episodes and new characters arriving and leaving constantly.
What it asks of you: a long commitment: each episode runs about fifty minutes, and the show rewards steady viewing across seasons rather than a single sitting or a quick sample.

Holby City1999
A sister show set in the same hospital universe, focusing on surgical wards rather than emergency. It extends the Casualty world with richer character continuity and higher-stakes operating theatre drama.
But It was cancelled in 2022, so you'll eventually reach the end. The tone can veer melodramatic, especially in later seasons.

Grey's Anatomy2005
American ensemble medical drama with the same blend of personal catastrophe and professional emergency. It has Casualty's structure but with more soap opera intensity and a massive archive to binge.
But It's ten times longer than Casualty and occasionally disappears into its own mythology. The sheer episode count is exhausting.

ER1994
The gold standard of emergency room dramas. Teaches you to love the chaos, the moral dilemmas, and the staff who hold it together. Influenced everything that came after, including Casualty's later direction.
But It's fifteen seasons and 331 episodes. The 1990s medical technology feels dated now.

The wildcard
Something you would not have found on your own.
M*A*S*H1972
Both prove that a hospital setting is fundamentally about the collision of human fragility and human competence. Character-driven ensembles thrive in emergency environments.
The blueprint for ensemble medical drama. Set in a Korean War field hospital, it shares Casualty's philosophy: chaos and incompleteness are features, not bugs. Staff save some patients and fail others; both matter. The show finds comedy in survival and meaning in small moments.
But It's a 1970s comedy-drama that aired as a serialized show, not a prestige drama. If you need contemporary production values, this will feel like work.

Scrubs2001
A hospital ensemble that uses humour to process genuine tragedy. It proves you can have both comedy and real stakes—messy, human, occasionally heartbreaking.
But The tone shifts after season five. Some viewers find the early comedy undercuts the drama; others love the tonal mix.

The Good Doctor2017
A more recent medical drama focused on the intersection of personal limitation and professional excellence. Modern production values and accessible storytelling.
But It can feel formulaic. The protagonist's autism is sometimes treated more as a character gimmick than authentically drawn.

House2004
Flips the medical drama formula. Instead of staff dynamics, it's the detective work of diagnosis—cerebral, tense, and utterly gripping. Different engine, same commitment to accuracy.
But After six seasons it gets repetitive. The protagonist is deliberately insufferable, which some find liberating and others exhausting.

Nurses2020
Canadian drama focused on the perspective of nursing staff rather than doctors. It corrects the imbalance in most medical TV and offers a genuinely different angle on emergency care.
But Cancelled after two seasons. Smaller cast means less of the sprawling ensemble dynamic.

St. Elsewhere1982
A 1980s hospital drama that Casualty was partly in conversation with. Rougher, more cynical, and fascinating as a historical artifact of how medical drama began.
But It's genuinely dated in tone and production. The pacing is slower than modern TV and some plots haven't aged well.

The Knick2014
Both interrogate medicine as a moral practice. What do we owe patients? What does the work do to the people performing it? The Knick just asks it at the turn of the 20th century.
A historical medical drama set in a 1900s New York hospital. Same structure as Casualty—rotating cases, staff drama, personal reckoning—but transforms the engine into a visual thriller. Cinemax production quality, restless editing, genuine historical intrigue about early surgery.
But Only two seasons before cancellation. Contains graphic medical procedures and drug use. The protagonist's cocaine addiction is central, not peripheral.

Over the Rainbow2015
Both love the NHS as an institution and the ordinary people who make it function. Casualty shouts about it; Over the Rainbow whispers. Same faith, different volume.
A short British drama (two episodes) about an NHS hospital radio station during a strike. Written by Ben Elton, it's a completely different register—smaller, quieter, more philosophically textured. Still about people holding systems together, but from the margins rather than the centre.
But It's a relic—BBC aired it once and buried it. Only 100 minutes total. Not a series, more like a long short film.
