Gina's Brother #1 turns up as exactly the kind of background menace that Big Mouth loves to toss into a scene: a sibling hovering at the edge of someone else's story, sizing up whoever's around Gina with blunt, proprietary suspicion. He doesn't need much screen time to make his presence felt.

The joke is entirely in the casting. Jason Mantzoukas has built a career on turning tiny, thankless roles into unhinged comic events, and a nameless numbered brother is exactly his kind of canvas — all bug-eyed intensity and threat delivered at top volume, gone before the scene even finishes.
